Prisoners' Families Survey 2018

Ormiston Families want to provide the best possible services for all those visiting a partner, parent, loved one or close family friend in prison.

To help us do this, we would really appreciate it if you could take some time to complete our survey questionnaire. If you would like help doing this, please ask an Ormiston Families member of staff at one of our visitors' centres.

To take part in our survey please download a questionnaire and either return a completed copy to our visits centre staff, or by email to enquiries@ormistonfamilies.org.uk.

Prize Draw

If you complete this survey you have the option to be entered into a prize draw in the New Year. To be entered into the draw, please add your name and contact details to the top of your completed questionnaire. (These details will not be used for any other purpose, other than to contact you if you win the prize draw. All survey information will be collated anonymously, and the original forms destroyed.)
